@OEWordhord Saying "mediaeval manuscript image" already avoids the "image image" problem (to say nothing of the "no alt text" problem!), so please don't feel bad. (P.S. Just to be clear, I'm not someone who relies on a screen reader, but I am someone who has discussed this specific question before, here and elsewhere, in order to try and write better alt text myself.)
