@OEWordhord I agree that "image" is redundant, but "drawing" or "illustration" would still be useful to know. So perhaps something like "Illustration of six cistercian monks singing at a lectern, from the corner of a mediaeval manuscript".
(May not actually be from a corner. I was just guessing.)

@OEWordhord Saying "mediaeval manuscript image" already avoids the "image image" problem (to say nothing of the "no alt text" problem!), so please don't feel bad.

(P.S. Just to be clear, I'm not someone who relies on a screen reader, but I am someone who has discussed this specific question before, here and elsewhere, in order to try and write better alt text myself.)
