    There ought to be something like the National Transportation Safety Board that forensically investigates data breaches-- not to identify the hackers, exactly, but to go through the whole decision tree that led to the exposure of customer/client data. AND, then there should be criminal penalties for executives if by their negligence or willful behavior, they contributed to the breach.
