I run GrapheneOS and most of the time I get notifications from Signal right away, but sometimes they don't deliver until I open the Signal app.
It's been like this with all my Pixels running GrapheneOS during the years.
Google Play Services was installed before Signal.
Both Google Play Services and Signal has unrestriced battery usage.
There is no restriction on data usage.
I have great celluar and wifi connection.
Reading different forums this might not be a GrapheneOS specific issue?
Does anyone have any input on this?

@seq I'm using the google play version. If not then you have to let signal pull notifications and that will use much more battery, right? -

@arutaz I'm not sure Signal offers a version completely without Play Services notifications. I'm not too knowledgeable about that though. But I never have more than single-digit battery usage with that app, which seems to corroborate that it isn't much different from the Play Store version. And compared to Home Assistant's "minimal version" app which requires running in the background and draws like 20-25% every bloody day...
