It was more than the fire radios, which were a symptom. The underlying problem was that in the long standing battle between the NYPD and FDNY, which every previous mayor stayed out of, he picked a side (NYPD, of course). And so they were allowed to exclude FDNY from the incident command center, resulting in no warning from the NYPD helicopter of the imminent collapse.
