I have a family member who has two different surnames on different documents. It causes an issue with the province of Québec when comes the time to deal with them; they don’t recognize the name used by Canada… only the one on their health card.

I've learned from experience that having different surnames on different important documents can prove complicated if you want to live in another country. However, if all of your documents match up, it's just very strange and disconcerting to learn about; no complications otherwise.

I’ve had zero issues with identification in life. Such an odd thing to find out so non chalantly on a Saturday afternoon in my mid 30’s.

It's not like you would have had many other records under the old name. Maybe if you ever needed to find & prove very early vaccinations?
