Whether it's because they were poorly written in the first place, or something changed and they no longer work, there's a refusal to deviate from 'The Process' like its some sort of religious observance. The inability or unwillingness to trust engineers to take appropriate precautions to manage risk or to know how to communicate effectively with the right people feels like a real failure of leadership.
